12 cm white diagonal crease and a 4 cm white crease line along with a couple of smaller creases below the top edge (right…slightly covered by the bar in my photo unfortunately)

two tiny tears and very mild creasing on the bottom edge including a 5 cm diagonal white crease line across the bottom left corner
